By the time John Picone finally sat across from Dr. Sanusi Umar, he had already seen nearly everyone.

Dermatologists. Primary care doctors. Specialists.

Each visit came with a new theory. A new cream. A new pill. A new promise that this time, maybe, the swelling would calm down. Maybe the infections would stop.

Some believed it was a simple scalp infection. Others thought ingrown hairs. Some called them inflamed scalp cysts.

But nothing worked.

The bumps kept growing. The swelling kept returning. The draining never truly stopped.

Because the real problem was deeper.

John was living with a combination of folliculitis keloidalis nuchae and chronic abscessing scalp inflammation. Two conditions known for painful bumps on the back of the head and recurring boils that rupture, drain, and return.

It was not simple.

It was layered.

And it had been progressing for years.

John Picone’s bumps on the back of the head showing the inflamed scalp cysts that has been growing for years.

The Lump That Drew Every Eye

When asked what disturbed him most, John did not hesitate.

It was the large lump on the back of his head.

He knew it drew attention the moment he walked into a room. He could feel it. Even if no one said anything, he felt the spotlight.

And when he lay down at night, the pressure would sometimes cause it to pop or drain.

Relief mixed with frustration. Then embarrassment.

But the lump was only part of it.

There were hundreds of boil-like bumps across his scalp. They would swell. Rupture. Fill again. A relentless cycle.

With every new flare, John felt more exhausted. More uncomfortable. Less willing to be around people. Eventually, he stopped going to places he once enjoyed.

It was not just pain.

It was isolation.

Dr. Sanusi Umar asks John about the most disturbing symptoms he has experienced having the scalp condition he has.

The Conversation That Changed Direction

When John met Dr. Umar, something shifted.

Instead of another quick prescription, there was a careful examination. A thorough look at the entire scalp. A direct explanation of what was truly happening.

For the first time, John heard the names of the two conditions working together to cause the swelling, drainage, and structural changes in his scalp.

For the first time, the failure of past treatments made sense.

Dr. Umar outlined a treatment plan aligned with established medical studies and tailored specifically to John’s disease stage. The focus was not just on calming symptoms temporarily, but on addressing the root cause of the painful bumps, the draining abscesses, and the ongoing inflammation.

It was not just another appointment.

It was understanding.

And understanding became hope.

After years of feeling trapped by a condition that seemed to have an endless grip on his life, John finally felt something different.

He felt possibility.

John stating that his scalp condition usually draws a lot of attention from people, making him uncomfortable in going out and socializing.

Some moments split your life in two.

For John, it was the day his car went off a cliff.

He survived.

But survival was only the beginning.

<img loading="lazy" decoding="async" width="1024" height="572" src="https://bumpinator.com/wp-content/uploads/2026/03/image-1024x572.png" alt="John’s cousin, Michael, explains how John’s mental health has been severely impacted by his current scalp condition.

When Trauma Lingers Beneath the Surface

After the accident, the emotional weight settled in first.

Depression. Isolation. A quiet sense that control had slipped through his hands.

Then came the physical changes.

Deep, painful lumps formed on his scalp. Boil like swellings that drained without warning. Thickened folds of skin that reshaped the back of his head.

What looked like simple bumps were not simple at all.

Over time, John was diagnosed with three distinct and serious scalp disorders: acne keloidalis nuchae, cutis verticis gyrata, and dissecting cellulitis.

Three conditions.

One scalp.

Years of compounding inflammation.

His cousin Michael described watching John withdraw from life. The pain was not just physical. It was the kind that makes you avoid mirrors. Avoid gatherings. Avoid being seen.

The Weight of a Triple Diagnosis

John came into the Bumpinator clinic with three current scalp conditions.

When John arrived at the clinic, Dr. Sanusi Umar, known to many as The Bumpinator, saw what others had missed.

Acne keloidalis nuchae causing keloid like nodules along the nape.

Cutis verticis gyrata creating deep, brain like folds across the scalp.

Dissecting cellulitis producing painful, draining nodules often mistaken for simple cysts.

Individually, each condition is challenging.

Together, they created severe deformation and recurrent boils that reshaped the back of John’s head.

This was not cosmetic.

It was chronic inflammatory disease.

And it had taken hold of John’s confidence.

The Turning Toward Restoration

Dr. U understood the complexity. More importantly, he understood the human being living inside it.

Treatment required careful evaluation, thoughtful planning, and surgical precision. The goal was not just to remove diseased tissue, but to interrupt the cycle of chronic inflammation driving the abscessing and scarring.

For John, this was the first time someone explained the science without dismissing the suffering.

For the first time since the accident, hope felt grounded.

Not abstract.

Earned.

Reclaiming the Back of His Head and His Life

Transformation did not erase what happened on that cliff.

But it did change the reflection staring back at him.

As the inflammatory masses were addressed and the deformity corrected, John began to lift his head differently. Stand differently.

His cousin noticed it first.

The depression that once defined his days began to loosen its grip.

Healing, in this chapter, was not just surgical.

It was personal.

And John walked away not just with a restored scalp, but with something deeper.

A renewed sense that his story did not end at the edge of that cliff.

<img loading="lazy" decoding="async" width="1024" height="572" src="https://bumpinator.com/wp-content/uploads/2026/03/image-2-1024x572.png" alt="A close-up look on John’s back of the head with deep cysts and boils on the scalp.

For sixteen years, Anwar lived with a weight few could see but he felt every day — raised, scarred lesions on his scalp known as acne keloidalis nuchae and folliculitis decalvans. Each morning was a reminder of the pain, the itching, and the questions from others that cut deeper than any scar. His scalp became a constant battleground.

When Anwar found Dr. Sanusi Umar, he wasn’t just searching for treatment. He was searching for relief, for a chance to look in the mirror and recognize himself again. What he discovered was more than a surgeon. He found someone who understood the emotional toll of the painful scalp spots and who had built tools from the ground up to fight it.

Removing a sixteen-year scalp keloids is no small feat. The scalp doesn’t easily forgive; it stretches little, and every millimeter matters. But Dr. U had designed something for this exact battlefield which is the Athena Suture Kit.

Invented in his own clinic, Athena allows surgeons to close large wounds under intense tension without distorting the natural shape of the scalp. It was made for cases just like Anwar’s where precision, durability, and beauty must live in the same line of sutures.

Dr. Sanusi Umar is removing a 16-year-old scalp keloids using the Athena Suture Kit during Anwar’s surgery.

The surgery demanded focus and faith for a successful treatment for acne keloidalis. Every stitch held not only skin; but hope that Anwar’s scalp could heal smooth, balanced, and whole again.

<img decoding="async" src="https://dru.com/wp-content/uploads/2026/01/unnamed-4-1024x591.png" alt="Close-up of the Athena Suture Guard medical packaging featuring a red cross logo, designed by Dr. Umar for high-tension scalp closures.
Close-up of the Athena Suture Guard medical packaging featuring a red cross logo, designed by Dr. Umar for high-tension scalp closures. Image caption: The Athena Suture Guard, designed by Dr. Umar for complex scalp reconstruction.

Weeks later, as the swelling faded, a new story began to surface. One of calm skin, quiet confidence, and relief. The wound had closed beautifully. The scalp had healed with harmony and strength. For Anwar, the reflection looking back finally felt like peace.

Healing gave him something surgery alone couldn’t — freedom. The freedom to move through life without hiding. The courage to show his journey so others might find theirs.

For eight long years, Adrian lived under the weight of a secret. What began as a small bump on the back of his head grew into a painful mass that shaped how he moved through the world. Every morning started with the same question: how to hide it. He pulled on beanies and hoodies even in the summer heat, desperate to blend in and avoid being stared at. Acne Keloidalis Nuchae, a chronic inflammatory scalp disorder, had not only scarred his skin but also his sense of self.

Adrian would hide the large Acne Keloidalis Nuchae (AKN) mass with beanies and hoodies, despite feeling uncomfortable in them.

At work, he stayed quiet, careful not to draw too much attention. He skipped photos, turned down outings, and invented reasons to explain his constant head coverings. The lies felt heavy, but the truth felt unbearable. Each curious glance reminded him that what he hid was more than a lesion. It was his shame, worn on his skin.

What people couldn’t see was the courage forming beneath the surface. Adrian’s struggle was not only to manage pain, but to protect dignity in a world that often mistakes appearance for worth. His story reminds us that real strength sometimes hides in plain sight, waiting for the moment to step forward.

Adrian opening up about the toll AKN took on his confidence.
