<p class="wp-block-paragraph">It began quietly.</p>



<p class="wp-block-paragraph">Small bumps appear along the back of the scalp. A little irritation at the neckline. Something easy to dismiss at first.</p>



<p class="wp-block-paragraph">But those bumps were not random. They were the early signs of chronic folliculitis of the nape, another name for acne keloidalis nuchae (AKN).</p>



<p class="wp-block-paragraph">This condition is an inflammatory disease that affects the hair follicles along the back of the scalp and neck. Over time, repeated inflammation can cause thickened skin, scarring, and clusters of persistent bumps.</p>



<p class="wp-block-paragraph">For John, this was the first trigger.</p>

<p class="wp-block-paragraph">When chronic inflammation develops around hair follicles, the body begins releasing chemical messengers into nearby tissues.</p>



<p class="wp-block-paragraph">These signals include cytokines and growth factors.</p>



<p class="wp-block-paragraph">They are part of the immune system’s natural response to injury or irritation. But when inflammation becomes persistent, these signals can begin affecting surrounding areas of the scalp.</p>

<p class="wp-block-paragraph">One of those conditions is dissecting cellulitis, a deeper inflammatory disease that can cause painful nodules, swelling, and sometimes drainage beneath the scalp.</p>

<p class="wp-block-paragraph">For John, the condition had progressed into <a href="https://bumpinator.com/what-is-acne-keloidalis-nuchae-akn/">acne keloidalis nuchae (AKN)</a> severe enough that surgery became the best path forward.</p>

<p class="wp-block-paragraph">After the procedure, John finally had a moment to breathe.</p>



<p class="wp-block-paragraph">Not long after, someone close to him walked in to see the results.</p>



<p class="wp-block-paragraph">His sister, Cheyenne.</p>



<p class="wp-block-paragraph">The moment she saw him, her reaction said everything. Relief. Excitement. Curiosity.</p>



<p class="wp-block-paragraph">“How did it go?” she asked.</p>



<p class="wp-block-paragraph">John explained that the surgery went well. In fact, the surgical team had to remove slightly more tissue than originally expected in order to fully treat the diseased follicles and scarred area.</p>



<p class="wp-block-paragraph">But the most important part was simple.</p>



<p class="wp-block-paragraph">The source of the problem had finally been addressed.</p>

<h2 id="h-when-one-condition-is-not-the-whole-story" class="wp-block-heading">When One Condition Is Not the Whole Story</h2>



<p class="wp-block-paragraph">During the episode, Dr. Sanusi Umar explains that John’s case was more complex than it first appeared.</p>



<p class="wp-block-paragraph">John was not dealing with just one inflammatory scalp disease.</p>



<p class="wp-block-paragraph">He had two.</p>



<p class="wp-block-paragraph">Alongside AKN, he was also experiencing <a href="https://bumpinator.com/bumpinator/what-is-dissecting-cellulitis/">dissecting cellulitis</a>, a deeper inflammatory condition that can create painful nodules or cyst-like swellings beneath the scalp.</p>



<p class="wp-block-paragraph">These conditions can sometimes occur together, and when they do, diagnosis becomes more challenging.</p>



<p class="wp-block-paragraph">In some patients, doctors may also see related disorders such as folliculitis decalvans, another inflammatory condition that damages hair follicles and leads to scarring.</p>



<p class="wp-block-paragraph">Recognizing these overlapping patterns was essential.</p>



<p class="wp-block-paragraph">Because treating one condition without identifying the other could leave the root problem unresolved.</p>

<p class="wp-block-paragraph">Many people see bumps on the back of their scalp and assume they are simple razor bumps.</p>



<p class="wp-block-paragraph">But conditions like AKN and dissecting cellulitis are very different.</p>



<p class="wp-block-paragraph">They involve chronic inflammation surrounding hair follicles, which can gradually damage the scalp and lead to permanent scarring if the disease continues unchecked.</p>



<p class="wp-block-paragraph">That is why careful evaluation matters.</p>

<p class="wp-block-paragraph">For John, it was the day his car went off a cliff. He survived the crash, but survival came with consequences that reached far beyond twisted metal. In the months that followed, depression settled in. He felt a loss of control, a quiet heaviness that reshaped his days and dimmed his sense of direction.</p>



<p class="wp-block-paragraph">What he did not expect was that his body would carry the trauma in another way.</p>

<p class="wp-block-paragraph">Not long after the accident, deep painful lumps began forming on his scalp. They looked like boils. They drained without warning. The back of his head slowly changed shape as thick folds and inflamed nodules developed.</p>



<p class="wp-block-paragraph">Over time, John was diagnosed with three distinct inflammatory scalp disorders: acne keloidalis nuchae, cutis verticis gyrata, and dissecting cellulitis. Each condition on its own can be physically and emotionally exhausting. Together, they created severe scarring, recurrent abscesses, and progressive deformation.</p>

<p class="wp-block-paragraph">Lying down became uncomfortable. Flare ups disrupted his sleep. The drainage was unpredictable. But the emotional toll ran deeper than the physical symptoms.</p>

										<content:encoded><![CDATA[<p><b>What is Dissecting Cellulitis? </b><span style="font-weight: 400;">Dissecting Cellulitis (DC), also known as Hoffman disease, is a chronic (long-term) inflammatory scalp disorder that causes abscesses and keloids that lead to patchy hair loss and </span><span style="font-weight: 400;">cicatricial</span><span style="font-weight: 400;"> (scarring) alopecia (destruction of hair follicles).</span></p>
<p><span style="font-weight: 400;">Dissecting cellulitis is incurable; however, somebody can manage symptoms to reduce pain and provide comfort.</span></p>
<p><span style="font-weight: 400;">DC is part of the follicular occlusion triad, where three diseases occur simultaneously. The other inflammatory diseases are acne conglobate (nodulocystic acne) and hidradenitis suppurativa (acne inversa).</span></p>
<p>The disease is most prominent in young adult black males but can, however, affect people of all races, ages, and sex.</p>
<h2><b>What Causes Dissecting Cellulitis?</b></h2>
<p><span style="font-weight: 400;">Specialists are working to determine the main cause of DC and establish why it affects some people and not others.</span></p>
<p><span style="font-weight: 400;">In this disorder, blocked hair follicles swell up and eventually rupture, causing inflammation underneath the skin.</span></p>
<p><b>Secondary Infection</b></p>
<p><span style="font-weight: 400;">The pus formed in the scalp following the follicle bursts becomes infected with bacteria over time, leading to further surface infections, hair loss, and scarring.</span></p>
<p><span style="font-weight: 400;">DC is often attributed as hereditary, but the disease rarely occurs in multiple family members. A small number of patients from the same family have experienced DC in the past, which leaves room for the assumption that genes play a role in the disease.  </span></p>
<h2><b>Symptoms of Dissecting Cellulitis</b></h2>
<p><span style="font-weight: 400;">DC, which starts as blocked hair follicles, worsens with time, with various signs and symptoms manifesting at the onset and later stages of the disease if not treated on time.</span></p>
<p><b>Early Symptoms</b></p>
<p><span style="font-weight: 400;">The most common onset symptoms include:</span></p>

<li><span style="font-weight: 400;">Swollen pimples on the scalp</span></li>

<h2><b>What Treatments Are Available for Dissecting Cellulitis?</b></h2>
<p><span style="font-weight: 400;">Once diagnosed, there are several recommended approaches to treating DC. As an incurable chronic condition, strategies have been put in place to manage active symptoms such as pain, hair loss, and lesions.</span></p>
<h3><b>Medications </b></h3>
<p><span style="font-weight: 400;">There are three kinds of supplements used to manage DC:</span></p>
